Following information needs to be furnished by you while intimating a claim:

  • Your contact numbers
  • Policy number
  • Name of Insured person who is injured
  • Date & time of accident
  • Location of loss
  • Brief description on how the accident took place
  • Extent of loss
  • Place & contact details of the Insured Person.

Overseas Travel Insurance - Important points to remember

Please notify your claim at Mayday Assistance call centre as soon as possible. In case you are not in a position to notify your claim, you may please take help of your relatives, friends, colleagues who can notify your claim on your behalf.

Medical Sickness/Accident

  • Notify Police authorities in case of Road Traffic accident
  • Please collect all bills and receipt and medical papers in original from Doctors, hospitals and other medical establishments.
  • Please get your claim form duly filled in and signed by the attending physician

Loss of passport:

  • Notify Police authorities immediately.
  • Please contact Indian Embassy for duplicate passport. You may take help of ISOS to locate Indian Embassy as well as know the formalities of obtaining duplicate passport.

Baggage Delay/Loss:

  • Intimate immediately about your loss to airlines.
  • Please obtain PIR report from the Airlines authority.
  • Please collect details of compensation received from airlines in case of baggage loss.
  • Trip Delay/ Cancellation/ interruption/ missed connection: Please collect Airlines correspondence certifying the cause of delay/cancellation

Financial Emergency/Bail bond:

  • Please notify Police authorities immediately
  • Please contact our service provider immediately.
  • Please contact Indian emergency if required

Personal Liability:

  • Do not commit any benefit/compensation or enter into any agreement.

Documents Checklist:

Medical Accident & Sickness Expenses** (Outside of country)
  • Claim form
  • Doctor's report
  • Original Admission/discharge card
  • Original Bills/Receipts/Prescription
  • Original X-ray reports/ Pathological/ Investigative reports
  • Copy of passport/Visa with Entry & exit stamp
Loss of Baggage
  • Claim form
  • Copies of Baggage Tags
  • Copies of Correspondence with the Airline authorities/Others about loss of checked baggage
  • Property Irregularity Report (obtained from Airline)
  • Details of compensation received from Airlines/Other authorities, if any
Delay of Baggage
  • Claim form
  • Copies of Baggage Tags
  • Copies of Correspondence with the Airline authorities certifying about the delay
  • Property Irregularity Report (obtained from Airline)
  • Original bills/receipts/invoices pertaining to expenses incurred/purchases made during the delay period.
Trip Delay/ Cancellation/ interruption/ missed connection
  • Claim form
  • Please attach confirmation from the airlines, clearly mentioning the scheduled arrival time and the actual arrival time
  • Copies of Correspondence with the Airline authorities certifying about the delay If trip is cancelled or interrupted due to medical reasons then provide medical reports and doctors statement
  • If trip is cancelled or interrupted due to employment reason, then termination letter from the company shall be submitted
  • If due to other insured events, police report confirming the incident shall be submitted In case the cancellation or interruption is owing to the sickness, injury or death of a traveling companion, the original tickets of the insured and the traveling companion indicating travel to the same destination for the same dates needs to be submitted All the bills / receipts of reasonable additional expenses incurred and / or proof of cancellation charges levied by the carriers shall be submitted.
Loss of Passport
  • Copy of new passport
  • Copy of previous passport (if available)
  • Original bills/invoices of expenses incurred for obtaining a new passport
  • Copy of FIR/ Police Report
Personal Liability
  • Full statement of the facts in writing
  • Witness statements
  • Any other documents relevant to the incident, including Summons, Legal Notice etc.
  • Any other information you would like to share with us.
Hijack
  • Full statement of the events in writing
  • Claim Form
  • Airline correspondence (copy of Passenger List etc.)
  • Copy of ticket/ Boarding Pass
Accidental Death & Dismemberment
  • Claim form
  • Original Death Certificate,
  • Copy of FIR/Police inquest report/Coroners report)
  • Copy of Post Mortem report.
  • Dismemberment Case:
  • Copy of treatment papers along with No. 1 &3 above
Financial Emergency Assistance
  • Date of loss

Bail Bond
  • Provide the court order stipulating the required amount as bail bond
  • Police report
Sponsor Protection
  • Provide medical reports, doctor's statement giving the details of the sponsor and cause of death certificate of the sponsor
  • Medical statements from relations / spouse will not be accepted
Compassionate Visit
  • Provide Medical reports and certificate from the doctor confirming hospitalization of the insured, reason of hospitalization and duration of hospitalization
  • Doctor's statement specifically stating the need for an attendant
Study Interruption
  • Provide Medical reports, statement from the treating doctor and death certificate as a proof of the reason of study interruption
  • Medical statements from relations or spouse will not be accepted
  • Provide receipts of the university fees paid
